Definitions of Florida’s medical professional license statuses.
Active - License is valid and the professional can practice in Florida.
Inactive - License is not valid and the professional cannot practice in Florida.
Authority void - The state voided the license, and the professional cannot practice in Florida.
Clear - Allowed to practice in Florida.
Conditional - The state has imposed conditions or restrictions on how the professional can practice.
Delinquent - License not renewed before expiration date.
Discp relinq - Professional gave up the license after the state took disciplinary action, and can no longer practice in Florida.
Emergency restriction - The state has restricted how the professional can practice in Florida, as a result of an emergency such as an allegation of substandard practice or criminal activity.
Emergency suspension - The state has banned the professional from practicing in Florida, as a result of an emergency such as an allegation of substandard practice or criminal activity.
Holds other license - The professional practices under another license.
Military active - A licensed professional now serving in the military.
Null and void - License has expired because the professional failed to renew it for two renewal cycles.
Obligations - The state has disciplined the professional by restricting the practice or requiring actions such as fines, community service or treatment.
Probation - The state has disciplined the professional by putting the practice on probationary status, often requiring supervision by another professional.
Revoked - The state has disciplined the professional by permanently taking away the license.
Suspended - The state has disciplined the professional by banning him or her from practicing in Florida for a certain time.
Vol-relinq - The professional gave up the right to practice in Florida, not as a result of a disciplinary case.
Volun withdraw - The professional agrees to withdraw from practicing in Florida.
VR pend bd act - Voluntary relinquishment case that is pending a board action.

Appeal affrmd/Revrsd/Remand Prt - Disciplinary action against doctor was appealed and reversed and/or the case was sent back to the licensing agency.
Appeal dismissed - Disciplinary action against doctor was appealed and dismissed.
Fine - Doctor was fined.
Fine and Letter of Concern - Doctor was fined and state licensing board wrote letter of concern about doctor’s actions.
Fine and Reprimand - Doctor was fined and reprimanded by state licensing board.
Limitations - Licensing board disciplined doctor by limiting his or her practice of medicine.
Obligations imposed - Licensing board disciplined the doctor by imposing restrictions or penalties such as fines, community service and treatment.
Penalty appealed/upheld - Disciplinary action against doctor was appealed and upheld.
Probation - The state disciplined the professional by putting the license on probation. State may require another profession to supervise the practice.
Reprimand - Licensing board reprimanded doctor.
Restitution required - Doctor ordered to repay the state.
Revocation - The state disciplined the doctor by permanently removing the license.
Suspension - The state disciplined the professional by banning him or her from practicing for a certain time.
Voluntary surrender - The doctor as part of a disciplinary case gave up his or her license and cannot practice.
